Most of the telehealth platforms for GLP-1 are medication delivery services in disguise as medical providers.
However, PlushCare does something that is truly unique: it functions similarly to an actual virtual primary care practice while simultaneously prescribing weight loss medications.
This singular difference makes nearly every aspect of the experience (better or worse) dependent upon what a particular patient’s specific needs are.
What Is PlushCare and How Does It Work?
PlushCare is a San Francisco based telemedicine company that provides full service virtual clinics prior to the common use of the term “GLP-1” for weight loss. It has now treated over 1 million patients with respect to primary care, mental health, urgent care, and weight management.
In addition, the physicians on its network are all Board Certified M.D.’s, rather than Nurse Practitioners, and they typically have same day appointment availability (most commonly in less than 30 minutes from when an appointment is booked).
For the purpose of treating weight loss, the process begins with a simple health questionnaire that’s designed to identify potential health issues that may be relevant to the treatment of obesity, including BMI. To qualify for GLP-1 prescriptions patients will need a BMI of 30 or greater, or a BMI of 27 if they also have at least one other medical issue related to their excess body fat.
Once a patient has completed this step and qualifies for GLP-1 prescriptions, a physician consultation occurs via a video call and the appropriate lab work will be ordered through Quest Diagnostics as deemed necessary. Once the test results have been received and reviewed by the prescribing physician, the prescription will then be electronically transmitted to the pharmacy of the patient’s choosing.
GLP-1 Product Offerings
Here is where PlushCare genuinely stands out from the crowd: the platform prescribes only FDA-approved brand-name GLP-1 medications, and it offers more of them than almost any other telehealth provider.
The lineup includes names like: Wegovy, Ozempic, Zepbound, Mounjaro, Saxenda, Victoza, Rybelsus, Trulicity, and Bydureon BCise.
Non-GLP-1 options like Contrave and Xenical are also available for patients who need alternatives. No compounded medications are offered, which is a deliberate policy.
PlushCare prescribes compounded semaglutide only when a brand-name product is on the FDA shortage list and only if the provider determines it is clinically appropriate.
Pricing: The Honest Breakdown
Membership fees with Plushcare will be $19.99 per month for unlimited platform usage and to send messages once you’ve finished your free 30 day trial. Additional consultation fees of $129 each are applied in addition to membership. Lab tests run at Quest Diagnostics will also have their own separate fee.
The costs associated with prescription medication will be handled through the pharmacy or insurance provider. If a person uses this program over a one-year period, has two consultations (a follow-up and lab test), and is able to obtain good insurance coverage for the GLP-1 medications as well as use copays, the total non-medicinal expense would be approximately $500.00.
Prior Authorization: The Real Differentiator
This is where the PlushCare model gets most of what we are talking about. PlushCare has its care team handle all of the prior authorization work with insurers. If the physician receives a denial on a prior authorization request, they have the ability to call the medical director at an insurance company to discuss and try to get overturned.
The physician being able to communicate directly (peer-to-peer) with the medical director at an insurance company is a very effective method of getting denials overturned, and this is something that does not exist with platforms that just direct users through alternative routes or methods once the insurance company says “no”.
Titration, Off-Ramp, and Non-Responder Protocols
Dosing options are determined by an individual board certified physician in collaboration with the patient and not by a billing algorithm. A patient will need to remain on medication for at least four weeks prior to escalating the dosage.
There has been no evidence that patients were forced to escalate their medication due to billing schedules. Plush Care does not have a formal tapering protocol nor do they offer a “maintenance only” membership option.
If a patient is unable to respond to Semaglutide there is a Tirzepatide pathway within the same physician-patient relationship and access to this can be achieved via the platforms’ broad formulary.

People who have good insurance coverage can usually work through a patient account (PA) process and end up really liking their specific physicians; many reviewers name particular physicians that kept them using the platform for years.
The common theme for negative reviews are billing issues from time-to-time, late appointment times by providers, and having the auto renewal of memberships surprise users.
In short, Plush Care was designed to serve insured patients looking for actual physician relationships and the best brand-name GLP-1 formularies available in telemedicine.
However, patients who are paying out-of-pocket will quickly find that the cost of services at Plush Care do not compare well financially to other options.
