Telehealth GLP-1 Review: Mochi Health

There is a particular type of GLP-1 telehealth provider that gets overlooked in most comparison articles: the one that actually bothers to put board-certified obesity medicine specialists in the room.

The majority of telemedicine platforms utilize non-board certified physician assistants or primary care physicians who have the authority to write prescriptions; however they may lack a comprehensive understanding of obesity as a chronic illness.

Physicians that are board-certified in Obesity Medicine (OBM) founded Mochi Health and continue to manage it…and the fact that these are physicians who specialize in OBM directly influences virtually every aspect of its operations.

What Is Mochi Health?

Mochi Health is a telemedicine company that is based in San Francisco and began their operations in 2022.

Mochi provides direct access to board-certified physicians and registered dietitian nutritionists (RDs) who provide comprehensive medical weight loss services using GLP-1 therapy.

Telemedicine consultations are provided in all 50 U.S. States as well as Washington D.C. The company has been awarded LegitScript certification; this means that an independent third party reviewed their website to meet certain standards regarding the validity of its information, honesty in advertising, and compliance with any applicable laws.

Additionally, there are very few other companies that are providing telemedicine weight loss services to adults and children alike via a separate pediatric division.

The physician network at Mochi includes multiple specialty providers including:

  • Obstetrician/Gynecologist
  • Bariatric Surgeon
  • Internal Medicine Specialist
  • Pediatrician, Endocrinologist
  • Pediatric Endocrinologist
  • Cardiothoracic Surgeon.

How Mochi Works

Eligibility is determined via an initial 5 minute online screening, after which patients schedule a virtual appointment with one of our board certified providers. Prior to the scheduled virtual appointment, the patient will receive a link to access the required intake documents, which are designed to collect information regarding current medications, daily dietary habits and previous medical conditions.

Following the completion of the virtual visit, the company will create a customized treatment protocol and, if deemed necessary by the provider, will send a prescription to either their partner compounding pharmacy or otherwise route it through the patient’s insurance carrier in order to be filled at a local commercial pharmacy.

A registered dietitian is assigned to each patient’s care team in order to provide nutritional counseling and support throughout treatment.

GLP-1 Product Offerings

Mochi has a large collection of prescription drugs for GLP-1 and non-GLP-1 medications that cover many different treatment options.

The list includes many GLP-1 medications, including:

  • Wegovy
  • Ozempic
  • Zepbound
  • Mounjaro
  • Saxenda
  • Victoza
  • Rybelsus

For people who have no health insurance, or for those who just want to use compounded medications, there are compounded versions of semaglutide (a GLP-1) and tirzepatide (a GLP-1) in both injectable and oral forms.

Pricing: What It Actually Costs

Mochi has an introductory membership price of $79 a month that includes all provider visits, Dietitian Access, Messaging, Care Coordination Support, Insurance Navigation Support, and Medications, which are separately billed.

The cost of compounded Semaglutide is a flat rate of $99 per month for every dose. This flat rate pricing across all dosages is a huge structural advantage over compounded Tirzepatide. The cost of compounded Tirzepatide will be a flat rate of $199 per month for all dosages, and again, this pricing will remain flat regardless of the Titration Level.

Shipping costs are also included with the purchase price of both medications. In other words, if you were to use compounded Semaglutide, you would pay $178 total per month ($79 Membership + $99 Compound Med) for compounded Semaglutide. If you used compounded Tirzepatide, on the other hand, you would pay $278 per month.

Prior Authorization: A Department, Not an Afterthought

Mochi has an entire department that is specifically for insurance navigation and prior authorization. Their own FAQ states that insurers have thirty days to respond to a PA, and that they will also work with patients to make sure that they receive medication coverage in the interim while waiting for the final approval.

The language around appeals is actually made explicit: Mochi handles “prior authorizations and appeals” as part of the membership.

Pharmacy Sourcing and the 503A Question

Although it doesn’t have its own pharmacy network (like Eden’s relationship with CloverX/Contigo, for instance), Mochi has partnered with a network of over 450 pharmacies which are vetted and are either 503A or 503B licensed. These pharmacies adhere to both USP 795 and 797 guidelines.

In addition to their own documentation of this partnership, there is also evidence available from the customer experience and other online communities where patients share unboxing photos and experiences with medications provided by Southend Pharmacy.

The benefit for Mochi using an array of pharmacies to send prescriptions through is: if one pharmacy runs into an issue due to inventory shortages )or some form of regulatory issue), then Mochi can continue to provide the medication to the patient without interruption.

Cold-Chain Shipping

Mochi also has its own direct answer to the cold chain issue in their FAQ. The company says that all of their GLP-1 drugs are good for 28 days or longer when stored at room temperature; they disclose that partner pharmacies test for stability (and therefore efficacy) under moderate temperatures during shipping.

Unboxing content from patients shows medication packaged in insulated boxes with gel packs. Although there is no public guarantee of an exact temperature controlled transit time window, Mochi does have more comprehensive information on stability testing disclosed to customers compared to many other competitors.

Titration: Provider-Directed, Flat-Rate by Design

The flat pricing model at every dose tier removes the structural incentive to push patients toward higher and more expensive doses.

For example, a patient who’s receiving .5 mg compounded Semaglutide will pay the same price ($99) as a patient receiving 2.0 mg compounded Semaglutide.

The physician makes the decision regarding how much medication is prescribed to the patient in conjunction with what the patient would like to do, through video visits that take place at intervals. Shared decision making (the process of the clinician and the patient deciding together) is positioned as one of the major clinical values of the platform.

Physicians who wish their patients to continue taking less than they may need to be able to afford are neither financially nor clinically encouraged or forced to increase their dosage.

Non-Responder Protocols

Approximately 15% of the patients that have a poor response to semaglutide may be able to receive Mochi’s tirzepatide as an option via the same medical practice and insurance coverage (formulary) as with their current compounded semaglutide.

A transition from compounded semaglutide to compounded tirzepatide can occur either during any scheduled doctor visits or by way of the physician directly communicating with the patient.

At this time, there are no defined criteria for triggering a switch from semaglutide to tirzepatide. Similarly, there are also no documented guarantees of achieving a certain amount of weight loss.
