Somer Schreiber-Small is a Family Nurse Practitioner (FNP-C), APRN, and board certified family nurse practitioner with 20+ years of healthcare experience is a medically supervised weight management program director. She collaborates with Dr. Vincent Covelli (MD), a Board-Certified Supervising Physician, and a Certified Nutrition Consultant to provide patients with an individualized and comprehensive approach to weight management and wellness.

At Synchronicity Health we utilize scientifically proven GLP-1 drugs in our medically supervised weight loss Program to assist with appetite control, metabolic health, and sustainably managing your weight. GLP-1 drugs are a prescription only treatment which is administered via weekly injections that work in harmony to provide your body with a natural way to control the urge to eat by minimizing cravings between meals; improving the sense of fullness after consuming meals; and helping to control portions.

The program begins with a comprehensive health evaluation performed by the providers, in which we will review the following: your medical history, lifestyle factors, current weight goals, and potential lab assessments. Following this evaluation, a personalized treatment plan will be provided to you, as well as continue monitoring, medication adjustments if necessary, and provide you with nutritional guidance based upon your results.

Gold Membership: $499/month or $5,988/year

Includes annual routine diagnostic evaluation, comprehensive physical, and expanded follow-up visits every six weeks with hormone and/or weight loss services.

Platinum Membership: $599/month or $7,188/year

Includes annual routine diagnostic evaluation, comprehensive physical, and expanded follow-up visits every four to six weeks with hormone and/or weight loss services.

Metabolic Health Reset Program: $799/month or $9,588/year

Includes routine diagnostic evaluation, comprehensive physical, follow-up, dietary plan, vitamin optimization; hormones; and weight loss services.
