Jacqueline “Jacki” Rodgers, MSN, CNP is the Clinic Director and head provider for Reform ABQ. As a board-certified Family Nurse Practitioner with over 15 years of experience providing primary care, integrative medicine, hormone therapy and wellness management, Jacki values her relationships with her patients and works collaboratively with them to create individual treatment plans that combine holistic and evidence-based medicine. Through her leadership skills and extensive background in the healthcare field, Jacki is committed to providing every patient with a pleasant experience, whether they come in for a routine check-up, hormone optimization, or metabolic support. Jacki has the support of the clinic’s professional staff, including Licensed Practical Nurses and Administrative Staff, to help her create a seamless experience for all patients.

Reform ABQ has medically supervised weight-loss and metabolic programs customized specifically for each patient’s health goals. They also offer GLP-1 medications such as semaglutide and tirzepatide, plus clinically proven peptides including; AOD‑9604, BPC‑157, CJC‑1295 w/Ipamorelin, and Ibutamoren.

Each patient starts with a complete medical evaluation (medical history, lab work, and lifestyle), which helps our providers establish an individualized program for that patient. These programs may include:

• Prescription peptide(s) and/or medication(s) when appropriate;
• Nutritional guidance to achieve optimal metabolic health; 
• Lifestyle and exercise recommendations;
• Ongoing provider oversight with continued follow-up to measure progress.

• CJC-1295 with Ipamorelin – $150.00 per month
• Ibutamoren – $150.00 per month
• Semaglutide (GLP-1) compounds – $280.00 per month
• Tirzepatide compound – $300.00 per month

 Actual price varies based on the amount of lab work required for each patient, dosage amounts, and unique treatment plans established during the initial consultation where the patient receives an estimate of treatment costs to help align the program with their health goals.
