Unlike many of the other GLP-1 telehealth platforms where you would simply be randomly matched with a healthcare provider and just hope for the best, Sesame Care allows you to search for your own board-certified provider, review feedback from previous patients, and even select the one that best fits your needs.

What is Sesame Care? And how does it operate?

Sesame is a telemedicine platform that introduced Success by Sesame, their dedicated weight loss program in 2024. Since then, the platform has allowed patients to connect with licensed providers to receive prescriptions for GLP-1, lab testing, and continued follow-up care across all 50 states.

As far as cost, the subscription is $99 every 28 days. This includes unlimited provider messaging via telehealth video calls, and laboratory testing through Quest Diagnostics. However, the cost of medication will vary depending on what you’re prescribed.

Additionally, a major partner with Costco allows Costco members to receive discounts on Wegovy injection prices at approximately $349 per month at Costco Pharmacy, which is about 50% off regular retail price. For the Wegovy pill, the cost begins around $149 per month.

How does Sesame offer GLP-1 products?

Sesame’s formulary is quite large. Through Success by Sesame, the program offers access to 14 prescription weight loss options, including brand name FDA approved GLP-1s: Wegovy, Zepbound, Ozempic, Mounjaro, Saxenda, Victoza, Trulicity and Rybelsus. Additionally, Contrave and Metformin provide alternative treatment options for patients who may need something different.

While Sesame primarily focuses on using FDA approved brand name drugs, compounded Semaglutide is used appropriately when possible, although it was initially priced at “cost” when Sesame first rolled out the program.

As such, Sesame tends to lean towards using branded options when they are made available due to insurance or manufacturer pricing.

How much does Sesame cost?

The $99 per 28 day subscription establishes a clinical relationship with the provider, but it also doesn’t include the medication itself.

If you have insurance and successfully obtain a prior authorization for brand name GLP-1s; these can often be purchased for copays as little as $25 per month. If you don’t have insurance (and if you are looking for competitive cash pay pricing), the Costco pharmacy partnership provides very good pricing for Wegovy injections.

It should be noted that the 28 day billing cycle vs. monthly billing is somewhat unique. As such, there are 13 billing cycles each year (versus 12), which results in an effective yearly cost of around $1,287 for the subscription alone (again, before medication costs).

Prior Authorization: Provider Driven Navigation

Sesame Providers help facilitate the prior authorization paperwork needed through insurance as part of the subscription. Unlike Ro or Calibrate (which use a dedicated team of professionals in order to navigate insurance appeals), Sesame uses a provider driven model.

This means that the level of prior authorization persistence will depend largely upon the individual provider you elect to work with (i.e. if you choose a provider that has significant experience with insurance).

This is both a feature and a drawback of the marketplace model. Patients who elect a provider with a high degree of familiarity with the insurance appeal process will likely receive higher levels of prior authorization persistence than those who elect a provider that is less experienced.

Titration & Clinical Flexibility

Since patients continue working with their selected provider(s), conversations regarding dosage occur in an ongoing clinical relationship as opposed to a simple transactional relationship between a patient and a revolving door of employees.

As long as a patient remains successful at a particular dose, they may remain at that dose without feeling pressured by bureaucratic considerations to increase the dose. Ultimately, however, the provider will set the titration schedule based on clinical success and not based on a billing calendar.

There is also no formal non-responder switching protocol outlined and no weight loss guarantee provided by Sesame. Tirzepatide is included in the formulary, so switching medications will be facilitated through clinical discussions rather than through protocols established at the platform level.

The fact that customers have varying experiences with different providers on Sesame due to its marketplace model creates variability that wouldn’t exist had customers utilized a platform offering a standardized care protocol.

Common complaints voiced by users consist of delayed responses from customer service representatives — along with difficulties some customers face when attempting to cancel their subscription and/or when they’re caught off guard by the 28-day billing cycle.
